The following information is for student athletes in grades 7-12 who will be a member of a SAHS team.

Is your Sports Qualifying Physical Exam Current?  

Sports Qualifying Physical Exams are good for three years from the date of the exam, but may not expire during the season for which you are registering.  Instructions on how to access your expiration date in PowerSchool can be found on Registration, Fees and Financial Assistance on the GoPonies website.

If your SQPE is NOT current:

1. Make an appointment with your doctor or go to a local walk in clinic to get a new SQPE.
    
2. Download a 2022-2023 blank SQPE form from SPORTS QUALIFYING PHYSICAL EXAMINATION (mshsl.org).  Take the blank form to your appointment.
    
3. When completed, scan/take a photo of the form and email to delahuntj@stillwaterschools.org
    
4. Allow up to 48 hours for data to be entered into the system.
    
5. To avoid registration delays, complete this process in July of 2022.
